1. Special Pre-flight Instructions Form This form is important information you need to know before visiting our center.
   
   
2. Mission Information Sheet This form must be completed and faxed to our center 1 week prior to your scheduled mission.
   
   
3. Emergency Closing Form
   This form must be completed and faxed to our center prior to your scheduled mission. In the event that the Challenger Center has to close due to inclement weather or other emergency, this form will allow us to notify you.
   
   
4. Directions To Our Center
   Please try to arrive 15 minutes before your scheduled mission.
   
   
5. Mission Patch Template
   In the tradition of NASA, each class that visits our center designs a patch to represent their mission. Teachers choose a student to design the patch or the whole class designs a patch, and the class votes on the patch. The patch is brought the day of the mission.
   
   
6. Applebee's Lunch Form
   Extend the day and satisfy your hungry astronauts at Applebee's Restaurant located next to our Center. We have made special arrangements with Applebee's to provide a selection of entrees for a reduced price.
   
   
7. Photo Release Form
   On occasion, we feature school groups in our promotional material. If you would like your class featured in our newletter, press release, website, etc., please download this form, complete it for each student, and bring the form(s) with your visit to our center.
   
   
8. Gift Shop List
   We have provided a partial list of items we offer in our center's store. If your class plans to use our gift shop, this gift shop list can be viewed prior to your visit to help your students make their selections quickly while they are at our center.
   
   
9. Gift Shop Request Form
   For your convenience and to save time during your visit, we have also provided a gift shop request form. Please complete the form and fax to us one week prior to your scheduled mission date, and we can arrange to have the items ready for you and your students the day of your mission.
