School Mission Forms
We want your students to get the most out of their experience at our center. We have provided the following forms to help make the most of your visit. Click on the form you wish to download.   1. Special Pre-flight Instructions Form
    This form is important information you need to know before visiting our center.

  2. Emergency Closing Form
    This form must be completed and faxed to our center prior to your scheduled mission. In the event that the Lower Hudson Valley Challenger Center has to close due to inclement weather or other emergency, this form will allow us to notify you.

  3. Crew Manifest Form
    The crew manifest allows you to arrange your class into Mission Control and Space Craft teams so we can expedite the preliminary activities before the class starts their mission. This form must be completed and brought with you the day of your mission.

  4. Mission Patch Template
    In the tradition of NASA, each class that visits our center designs a patch to represent their mission. Teachers choose a student to design the patch or the whole class designs a patch, and the class votes on the patch. The patch is brought the day of the mission.

  5. Team Descriptions
    Prior to the mission, the teacher creates the crew manifest, since the teacher is familiar with students' personalities and abilities. Each student is assigned a partner on one of eight teams. This document is a brief description of the eight teams.

  6. Directions To Our Center
    Please try to arrive 15 minutes before your scheduled mission.

  7. Applebee's Lunch Form
    Extend the day and satisfy your hungry astronauts at Applebee's Restaurant located next to our Center. We have made special arrangements with Applebee's to provide a selection of entrees for a reduced price.

  8. Photo Release Form
    On occasion, we feature school groups in our promotional material. If you would like your class featured in our newletter, press release, website, etc., please download this form, complete it for each student, and bring the form(s) with your visit to our center.

  9. Gift Shop List
    We have provided a partial list of items we offer in our center's store. If your class plans to use our gift shop, this gift shop list can be viewed prior to your visit to help your students make their selections quickly while they are at our center.

  10. Gift Shop Request Form
    For your convenience and to save time during your visit, we have also provided a gift shop request form. Please complete the form and fax to us one week prior to your scheduled mission date, and we can arrange to have the items ready for you and your students the day of your mission.   11. School Mission Documents Zip File
    For your convenience, we have compressed all the above PDF files into a zip file that you can download to your computer for quick and easy access.
